Consolidated Tax Voucher not Available

What information do I need to provide if my broker does not issue a UK Consolidated Tax Voucher. I have an account with Interactive Brokers and the account is managed by a Fund Manager but IB does not issue a Consolidated Tax Certificate. I have a statement of the dividend payments. What other information do I need to provide?
