HMRC are aware that some customers holding a Frontier Worker permit have been unable to apply for an A1 certificate online and apologise for any inconvenience caused.  
We updated the CA8421 application form on 11 November and it can now be used by all Frontier Worker Permit holders. 
Following a recent process change, frontier workers seeking to apply for, or renew, an S1 certificate need to apply for an A1 certificate using form CA8421. They will automatically receive an S1 certificate (for themselves) alongside their A1 certificate if they are eligible.

The CA8421 does not issue S1 certificates for dependent family members. If any dependents of the individual require S1 healthcare certificates, customers should apply for these using form CA8454.

I’ve been renewing my S1 online through the HMRC website for 10 years. While there were occasional issues, the process always worked eventually. This year, however, my foreign insurance agency offered to renew the S1 on my behalf—and after three months of silence, nothing has progressed. HMRC hasn’t even clarified whether the process has changed. The lack of information is frustrating. Thanks to this forum, I’ve discovered that I now need to complete form CA8421, though its title is misleading (the UK doesn’t cover healthcare if a significant amount of work is carried out abroad). For dependents, it seems CA8454 is required, and its title is equally confusing (suggesting eligibility only in cases like maternity leave). This situation is a disaster. My S1 has already expired, leaving us without healthcare coverage.
