Which IHT form for managed investments?

I'm starting to fill in the forms for my late mother's estate, and it's pretty clear where to give details for her bank accounts and shares, but there's one other source of income that I'm finding hard to place. She also had investments managed by a financial advisor; we don't have an account and they are not company shares, so where do I enter those details?
