Underpayment on PAYE

Hello, I have been in the same job over 10 years under PAYE and with no changes in employment or benefits in the tax year 23/24. Yet I am being asked to pay 5.5k for underpayment of tax. Given no changes in circumstances, how can the tax collected be so wrong? Thanks in advance for any help
