Do tax-exempt payments from the employer on account of disability pursuant to section 406(b) of the Income Tax (Earnings and Pensions) Act 2003 (ITEPA 2003) need to be reported in self-assessment tax returns?

No you won't include them as taxable income. You can make reference to them in the comments section.
