Proof of Identity for registering for Government gateway

My mother (who is over 100 years old ) is now required to submit a self assestment tax return for the first time. I have been trying to help her to get a government gateway set up but she does not have any of the evidence required to prove her identity . How should we proceed . She now has a UTR .
