Self Assessment Tax Bill

I recently received a £75 penalty notice for not paying overdue tax on time and this was surprising. When I submitted by last self-assessment tax form in May 2022, it said that HMRC would try to collect any underpaid tax through my tax code. There was a box to tick if you did not want to do this and I did not tick it. I meet all the criteria for that to happen. Do you know why this process hasn't been followed this time?
