I am a self employed freelancer and a client has offered to include me in their health insurance plan even though I am not an employee. Does anyone know whether I need to declare this on the self assessment form and if so where on the form this would go given that I am not an employee? I'm wondering whether this section only appears if you tick Yes on the Employment section of the form.

Yes, you would need to include this and the value of the benefit would need to be added to your turnover.
