Transfer of property from solely in wife's name to joint names

I am currently trying to complete the AP1 Application to change the register form. I am unsure what to list under "Applications in priority order" section 4. Section 5 Documents lodged with this form - would this just be the Identification forms? Which sections need to be completed for the transfer of title into joint names? Thank you
