I have an EIS5 form which on page 1 shows a list of 13 different companies in which I have invested through the approved fund set out on the same page. I now wish to claim deferral relief using the form for investments I sold in a previous tax year for which I have already submitted a tax return. However, on page 2, section 2 Claim to EIS Deferral Relief only allows me to enter one company name (in the box labelled "Shares in."). Should I therefore send 13 copies of page 2, each filled in with one of the companies in which I have invested, to HMRC? Or if not, what should I do?

You can attach a spreadsheet/letter to claim the relief on the other sources.
