Declaration of trust template for rental income beneficiary

Dear HMRC, I and my wife own a property jointly, I would like to make declaration of trust so my wife becomes 100% beneficiary of the full rental income. Can you please help with template or example of the declaration of trust to make this declaration, also please confirm if the declaration needs to be sent to HMRC via Form17 ? Thank you.

As the property is jointly owned you will have to go through the Form 17 process.  
We do not provide templates for declaration of trusts.
