Requesting taxable income/PAYE over the past 15 years

I'm trying to find out how to put in a request to receive the total taxable income for the last 15 years - I understand this is possible but am not sure where to find the correct advice/form. This is linked to a rebate under the Goodwin Ruling for Teachers Pensions. Many thanks in advance for any advice.
