SDLT form or certificate for transfer of share of property

I need to transfer my share of the former family home to my ex following divorce and financial settlement court order. I have received money for that share. I understand that stamp duty is not due because the transfer is part of the court order, not an actual sale. The Land Registry says I still need to file an SDLT form from HMRC with our request to update the Land Registry and take my name off. What is that form and where do I find it? Thank you
