Claiming PPI tax paid - template letter

Hi I had to submit a direct letter for back dated tax refund 1st November 2023 as unable to complete a R40 as I had already submitted a self assessment for years 2019/2020 - received a reply 1st October 2024 later to say that the claim was not in a specific format I have read the manual etc but please does anyone have a template of a letter as struggling to know what I have left out many thanks
