Living in Portugal / work remotely for a UK company

Hi, I’ve recently moved to Portugal but work remotely for a UK company as a employee. I have received a double-taxation certificate from my accountant in Portugal. Where do I need to send this and are there any other forms I need to complete?

Please send this to:
HMRC,
PAYE & Self Assessment,
BX9 1AS.
You will need to include the Portuguese residence certificate with the form.
