How to get the self assessment tax return in paper form

How to request self assessment tax return in paper form (SA100 & supplement form) except making a phone call ? Can I use other ways to request such as email ? When requesting the paper forms, is it necessary to request the specific forms that I expect to use ( such as SA100, SA106, SA109, etc...) ?
