I applied for the UTR on the 8th Sep but haven't received the number. I called HMRC this morning, the agent just asked me to submit again. When i asked what's wrong with my application (so that i can avoid mistake when i apply again), the agent just replied "i can't comment". Could someone please advise me what to do? Thanks!

If you've already registered for Self Assessment you can find out when you should receive your UTR:
Check when you can expect a reply from HMRC

Same situation. I applied a few days ago but the expected response time is in November which passed the deadline. What if the application is rejected, will I be fined for late registration?

If the timescale has passed to receive your UTR then you will need to contact HMRC to review:
