Which form to fill in for transitional basis period for husband and wife partnership

Hi I have sent off my short partnership tax return, but now need to fill in info for the new transitional period. It is a husband and wife partnership with under £20000 turnover. Please could you let me know which form or forms we have to fill in to provide this information
