Personal Details does not match HMRC record when applying self assessment

I have been applying the self assessment for at least 5 times to get the UTR number. But I am rejected for every application with the reason of "We could not process your application because your personal details do not match our records." I have checked my personal details in my Personal Tax Account and the information I filled in the self assessment form, which the personal details are aligned and correct. Not sure if anyone has faced the same issue before, and how you sort it at last?
