Late Payment Penalty Despite Successful Direct Debit Setup for Tax Payments

I set up a direct debit for monthly tax payments last October and successfully paid each installment since November (having paid four installments to date). Has anyone experienced being penalized with a 30-day late payment fee despite making several tax payments? I submitted an appeal application at the end of last month, but today I received another penalty notice.
