Hi - my daughter is 16 and has received her national insurance card through. Its in the wrong name as she changed her name a few years back. We've tried to registed on the portal to complete the name change on line but she only has a passport so not enough ID. I did speak to someone a few months back who told me which document she needed to download and complete but I cannot locate it now. Can someone point us in the right direction ?

Hi,
Your daughter should call the NI helpline on 0300 200 3500. Lines are open 8am – 6pm Monday to Friday.  
Please note that whilst obviously you can assist her with the call if she wishes, your daughter must make the call herself.

However please note, the area that would deal with her written request are currently dealing with work which was received in March 2023.
