  • RE: P11D form (Private Medical Insurance) provided by my employer.

    In this scenario, I have a question. I found that there are three columns against "Private medical treatment or insurance" in my P11D. 1. First column is "Cost to you or amount foregone" amount enter in the first column is xxxx.xx 2. Second column is "Amount made good or from which tax deducted" amount enter in the first column is xxxx.xx 3. Third column is "Cash equivalent or relevant amount" amount enter in the first column is 0.00. Third column is mark as final amount. The entire "Private medical treatment or insurance" section look as a part of equation. The equation looks like "Cost to you or amount foregone" - "Amount made good or from which tax deducted" = "Cash equivalent or relevant amount" What shall I mention in my tax refund "xxxx.xx" or "0.00"?
