EIS Form

Dear HMRC, I would like to claim tax relief, on the first page of EIS form, there is : Name and address of the person who subscribed for eligible shares. Ok that's fine but has my old address, should I write the new address or do I have to keep it with old address ? And on the page 3 I will write mu current address as usual. Thank you very much
