I am a Uk resident and have lived here since 2015. My home country, Portugal has requested this model E104/S041 as proof of NI contributions here from that date and want to fine me for not having submitted and hence missed contributions in Portugal, since that date. My understanding is that I can't request this form but it would have to be requested by them to the UK. Is this correct? What do they have to do as it seems they don't have the correct information (including lawyers)? Thank you.

Hi Elsa Miranda,
Your understanding is correct, the authorities in Portugal will need to apply for the E104/SO41.
They can request this from HMRC by completing form S040.
