I will be making a pension payment this week of £15k net. The gov website says "If you’re paying in an amount greater than £10,000, you’ll need to contact HMRC to claim the tax relief." Does this also apply if I do self-assessment? Or can I just declare it there and add the evidence as an attachment?

You can include it on the tax return and attach the evidence as a PDF document.

Thank you. And for the evidence, is a screenshot of my pension portal sufficient, showing additional payments with dates and tax relief for each payment?

Yes this would be acceptable as long as your personal details can also be seen.
