Could someone please tell me how to fill in the assessment form? I did an one-off contribution to my private pension of £8000 and receive £2000 tax relief. In the assessment form, there are two text boxes. Text Box 1: Payments to registered pension schemes (also known as PPR) where basic rate tax relief will be claimed by your pension provider (called 'relief at source') Text Box 2: Total of any 'one-off' payments to registered pension schemes included in the 'Payments to registered pension schemes where basic rate tax relief will be claimed by your pension provider' box What value should I put in the text box 1 and the text box 2? Besides, I assume that my workplace pension and my salary sacrifice (deducted from my salary) are not required to be entered into the assessment form. Is that right?

Hi Simon Yuen,

The amounts deducted from your salary should not be declared.
For the lump sum amount plus the relief should go in box 1.
