Maternity Allowance - Self Emplyed + Unpaid CIC Director

Hello I have been working and registered as self employed since April 2024 and I am expecting a baby in February 2025. I was employed full time until March 2024. I am also an unpaid director of a CIC (community interest company). As an unpaid CIC director, do I need to complete an SMP1 form for myself with the Maternity Allowance Claim Form? Many thanks
