If you were employed then no form needed as the employer would submit the pay anf tax details to HMRC.
If self employed you would need to register and complete a Self Assessment.

If you are self employed you can contact the self employed helpline 0300 200 3500 or download and submit the CWF1 form by post.
If not self employed then you can download and complete the SA1 form by post.

If you update your address online we do not automatically send out a change of address letter.
If you have updated your address then any correspodence will now go to the new address going forward.
